LIABILITY RELEASE: I have investigated and hereby assume all risks involved with serving in any capacity for and/or at Regency Christian Center International, including property damage, personal or bodily injury, or death. By signing this document, I hereby release and forever discharge the Regency Christian Center International, its officers, directors, employees, contractors, agents, and any parties volunteering on behalf of the Regency Christian Center International (the “Released Parties”) from all claims, damages, costs or expenses of any kind arising out of or related to the my attendance, participation, or serving in any capacity for and/or at Regency Christian Center, including those caused by the Released Parties’ negligence. I understand that this is a full and complete Release of all claims for property damage, personal or bodily injury, or death which might be sustained as a result of such attendance, participation, or serving in any capacity for and/or at Regency Christian Center, regardless of the specific cause thereof, and I agree that in the event of such injury or damage I cannot and will not seek any type of recovery or reimbursement whatsoever from the Released Parties. Further, I agree to indemnify and hold harmless the Released Parties from any liability, loss, or damage that may be incurred by me, whether caused by the negligence of the Regency Christian Center or otherwise.

Should I become a Regency Team Member, I agree to read and uphold the Regency Team Member Handbook guidelines and standards to the best of my ability, I agree to lead a lifestyle that honors Christ, and I agree to model refrain from a description contact conduct in the performance of my service on behalf of the church. The information contained in this application is correct to the best of my knowledge. I authorize the release of this information to Regency Leadership, Staff, and decision makers. I further state that I have carefully read the foregoing Liability Release and know the contents thereof, and I am checking the “Yes, I am excited to become a Regency Team Member” box, I affirm that I agree to this release as an act of my own free will. I have read and agreed to all terms and conditions.
